Hello, come on in and check out the stuff! Certifications in the Salt Lake City and Ogden, Utah areas; diving guidelines; altitude table; humor; mermaid encounter; photos. The SCUBA Diving guidelines and the Altitude Conversion Table are for use by certified divers. Those uncertified might learn more about diving by looking at them, however, only certified divers should be using them.

Do not dive unless you have been certified by an instructor.

• Student Medical Statement Forms. The Medical History Form must be filled out by all students. If all the answers to the questions are "No", then that is all that is required. If any answer is "Yes", then a doctor's examination is required and the student and the doctor must fill out the Second Medical Form.
1. Medical History Form: Must be filled out by all students before the class. Give yourself enough time before the class in case a doctor's exam is necessary.
2. Second Medical Form: Required when a doctor's examination is necessary from a Yes answer on the Medical History Form.
